2009 YOUTH GRANT REPORTS:

Click here or on the image below for a PDF file with everything you need to submit your 2009 Youth Grants report. It's really simple, just one page, so no worries!

 

REMEMBER: submit your report by 6:30PM Thursday, November 19th, you'll be entered in a prize draw! The final deadline to submit your report and receipts is December 31, 2009.

 

Here are your options:

  • Print out the PDF, fill in your answers, and fax or mail it to us (our contact info is on the form and below) OR
  • Click here to fill out the form online (but read this first!) OR
  • Make a video with your answers to the report and send it to us by mail or e-mail (but read this first!) OR
  • Call us at 461-3073 and we can interview you over the phone to get your feedback (but read this first!) OR
  • Come to the YOUTH GRANTS CELEBRATION, November 19th from 5:00-7:30 at the United Way office and fill out the form on site!

For all of these options, don't forget to send us your receipts (copies or originals) by mail or fax to:
